On 3 October 2026, Princess Caroline of Hanover attended the fifth edition of Smile Day (Journée du Sourire), a charity event held at Anne-Marie Campora Square in Larvotto, Monte Carlo, Monaco. The event, organised by the Les Smileys Monaco association, aims to support children as well as their families by providing comfort during stressful and anxiety-inducing moments related to medical care.

Chaired by Joëlle Bacrot-Bardos, the Les Smileys Monaco association has 42 volunteers who work across various healthcare facilities in the Principality, including the Princess Grace Hospital Centre (CHPG), the Cardio-Thoracic Centre, and with the fire and rescue services. For the occasion, Princess Caroline wore a palm-green suit by Blaze Milano.
